Position: Nurse Practitioner - Geriatric Primary Care

Location: Greater Oakland area

Salary: $129K $207K (DOE) W2

Schedule: Full-time; Monday-Friday 8a-5p

Duration: Permanent

Seeking a Nurse Practitioner to join a PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team serving an adult and elderly population (ages 55+).

Position Highlights:

• Provide adult Primary Care in a value-based model
• Seeing an average of 5 10 patients/day in a clinic setting
• Manage acute and chronic conditions, conduct assessments, and contribute to team-based care plans

Qualifications:

• CA NP license (FNP, AGNP, or ANP), Furnishing License, DEA, CPR
• Open to new grads; experience in geriatrics or family practice a plus
• Must have at least 1 year of recent Primary Care experience
• Flexibility to work across 3 locations within an assigned territory
