Job Description Patterns Hiring is seeking a travel Home Health Physical Therapist for a travel job in Clearlake, California. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: 04/13/2026 - Duration: 12 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Clear Lake, CA | ? 13 Weeks | ?? 40 hrs/week – 8:00a–4:30p (Mon–Fri) - Seeking an experienced Physical Therapist – Home Health with 2 years experience in home health settings. - Must be proficient with OASIS documentation and Homecare Homebase (HCHB). The PT will provide in-home evaluations, treatment plans, and patient-centered care, focusing on improving mobility and functional independence. - Must have a valid driver’s license and be comfortable traveling between patient homes.
