Job Description Towne U.S. LLC is seeking a travel Home Health Physical Therapist for a travel job in Wall Township, New Jersey. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel About the Role We are seeking an experienced Home Health Physical Therapist (PT) to provide professional rehabilitation therapy to patients in their homes. The PT will complete evaluations, develop individualized treatment plans, and deliver goal-oriented therapy programs in accordance with physician orders, organizational policies, and Medicare home health standards. Key Responsibilities - Complete comprehensive patient evaluations within required timeframes reflecting a holistic assessment - Develop individualized treatment plans with functional, measurable short- and long-term goals - Provide skilled therapy services in the home following the physician’s plan of care - Prepare and submit timely documentation, including progress notes, reassessments, and discharge summaries - Ensure documentation clearly supports the need for continued skilled home care services - Monitor patient progress and adjust treatment interventions as needed - Coordinate care and act as liaison among physicians, care managers, and interdisciplinary team members - Supervise Physical Therapist Assistants (PTAs) and complete required co-signatures - Provide written home exercise programs and education to patients and caregivers - Participate in discharge planning and ensure summaries reflect outcomes and recommendations - Maintain scheduling updates, respond to messages promptly, and document case communication appropriately - Meet productivity expectations while maintaining high-quality patient care - Perform duties requiring lifting/pushing/pulling up to 50 lbs and extended standing Required Qualifications - Bachelor’s Degree in Physical Therapy (Doctorate preferred) - Registered Physical Therapist license (NJ or eligible) - Minimum 1 year PT experience required - Certified Medicare Home Health experience required (Part A) - OASIS assessment experience and current usage REQUIRED - BLS certification (AHA) required - Strong PT evaluation and intervention skills - Ability to multitask and complete documentation accurately and timely - Excellent communication and organizational skills Preferred Qualifications - Prior Home Health experience strongly preferred - Experience using EPIC documentation system preferred Towne U.S. LLC Job ID 17859934.
