Job Description We are seeking a Board-Certified Primary Care Physician to join a full-time, outpatient-focused practice in San Jose, CA. Position Overview - Role: Full-Time Physician – Primary Care - Specialties Accepted: Family Medicine, Internal Medicine, or Pediatrics - Location: San Jose, CA - Practice Setting: Outpatient clinic Clinical Responsibilities - Provide comprehensive primary care services to new and established patients - Perform physical exams, preventive visits, and health maintenance care - Diagnose and manage acute and chronic conditions - Order and interpret diagnostic tests and develop treatment plans - Educate patients on disease prevention, wellness, and self-management - Coordinate care with specialists and facilitate referrals when needed - Maintain accurate documentation using Electronic Health Records (EHR) - Participate in quality improvement initiatives and peer reviews Team-Based Care - Collaborate with a multidisciplinary team including behavioral health and wellness professionals - Support integrated care addressing medical, behavioral, and community health needs - Supervise and mentor APPs, residents, or medical students as appropriate ? Schedule & Coverage - Full-time outpatient schedule - Rotational after-hours call coverage - Coverage for colleagues as needed ?
